Affiliated Player Playoff Request Form
This form is to be used when requesting to use an affiliated player in a league playoff game.
It will be filled out by the team that is wanting to use the affiliate and sent to the governor of the team that wants to use the affiliate.
You still need to have permission from the full time team to use this player prior to filling out this form.

If you do not see a "Success" page pop up after you hit send, you may have to re-enter the verification number again or an error may be displayed in Red.
